How can I get a duplicate SKA card?

A duplicate SKA card is issued in the case of loss, theft or damage of the previous card. It is possible to receive a duplicate only in the case of a personal card.

Procedure on the case of loss or theft

  1. File file an application for blocking the card (file format: PDF – file size: 278 kB) at the Customer Service Point or at the Customer Service Office (these are selected ZDiTM ticket offices), or via the website.
  2. Fill in and file file an application for issuing a duplicate SKA card (file format: PDF – file size: 275 kB) at any Customer Service Point or Customer Service Office.
  3. The duplicate is issued immediately after blocking the previous card, confirming the data on the card and the e-wallet balance.

The fee for issuing a duplicate SKA card is 10.00 zł.

Procedure on the case of damage

  1. File file an application for blocking the card (file format: PDF – file size: 278 kB) at the Customer Service Point or at the Customer Service Office (these are selected ZDiTM ticket offices) and return the damaged card.
  2. Fill in and file file an application for issuing a duplicate SKA card (file format: PDF – file size: 275 kB) at any Customer Service Point or Customer Service Office.
  3. The duplicate is issued immediately after blocking the previous card, confirming the data on the card and the e-wallet balance.

The fee for issuing a duplicate SKA card is 10.00 zł. The card can be replaced free of charge if a manufacturing defect is revealed.
